I’m Malori Maeva - the founder and creative director here. Form began as a one-woman operation in my second-floor walkup 12 years ago. In that time, we’ve created floral designs for thousands of weddings, corporate events, styled shoots, and so much more.

Based in sunny Phoenix, Arizona - my team and I have extensive experience working alongside our incredible desert landscape to create jaw-dropping moments that accent our clients’ biggest days.
